PHOTOGRAPH AND VIDEO RELEASE: By checking I agree to the following disclaimer the freelancer, employee, guest, and spectator agrees to grant permission to WWE (World Wrestling Entertainment), Wrestling Federation of America, Elite Pro Wrestling Training Center and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or videos and images taken of me, or members of my family, for the purpose of publication, promotion, product sale, profit and gain, illustration, advertising, or trade, in any manner or in any medium. I hereby release WWE (World Wrestling Entertainment), Wrestling Federation of America, Elite Pro Wrestling Training Center and its legal representatives for all claims and liability relating to said images or video. CONSENT RELEASE: I, the undersigned, hereby authorize Elite Pro Wrestling Training Center to conduct a background check as part of my application for employment, training, or participation in any related programs. This background check may include, but is not limited to, inquiries into my criminal history, employment history, education verification, and any other information necessary to evaluate my suitability for this position. I understand that: I have the right to receive a copy of any report generated and to know the source of the information. If any adverse action is taken based on the information obtained, I will be notified and given the opportunity to dispute the findings. The information obtained will be used solely for the purpose of assessing my application and will be treated confidentially.
